
Referral Process: This process is very simple. Anyone can make a referral (the caretaker or the referral source). A referral form is completed to gather information about the child's presenting problems and symptoms. This referral information is then assigned to a licensed clinician for an initial screening to determine if all eligibility requirements appear to be met. Once a decision is made as to whether or not the agency will accept the case for an assessment, the caretaker and/or referral source is contacted. If the case is accepted, an appointment will be set for the child to see the agency psychiatrist for a medical review/ assessment and also an appointment with the licensed therapist to complete the Assessment Form. Records are requested during this time to verify that “duration” requirements for eligibility are met. Once all of this is complete and if the child continues to appear to meet criteria, then the Assessment Packet is submitted to the Office of Mental Health who makes the final decision regarding eligibility (responses are received within five business days). Referrals can be made at any time during our business hours Monday – Friday between 9am-5pm either in person or by phone.
